Registration Status in the Centralised Organ Donation Register (CODR)

How to find out your registration status

1. Online

   • If you are an "iAM Smart" user, you can:
   • know your registration status instantly - click the "Continue with iAM Smart" button

   • apply to have a copy of your registration information by submitting an online form - click Data Access Request (DAR).

2. By post

   • If you are not an "iAM Smart" user, you can submit a DAR form in paper format, enclose a copy of document(s) of identification, and return them to the CODR Office by post: the CODR Office, Department of Health, Units A-D, 8/F, China Overseas Building, 139 Hennessy Road, Wan Chai, Hong Kong.

3. In-person application

   • Please bring your Hong Kong Identity Card for submitting a DAR at the CODR Office.

   Note: a DAR report will not be rendered immediately on the spot.
