Registration

Please READ these Notes for applicants and Personal Information Collection Statement before you provide any personal data to us.

Notes for applicants:

  1. If you cannot provide the requested information, the DH may not be able to process your application.

  2. If you choose to apply for registration in-person or online, identity verification can be completed up front during the application process. If you apply by post or fax, upon receiving your application, staff of the DH will process your application. Upon completion of your application for registration, your wish to donate organs after death will be recorded in the CODR.

  3. In general, for applications submitted online, the registration process will be completed in a shorter time. If you submit applications in person, by post or fax, you will receive a written notification within 14 days after submission of your application. If you do not receive the written notification, please contact the CODR Office of the DH by phone on 2961 8441 / fax on 2127 4926 or by email at codr@dh.gov.hk for enquiry.

  4. After successful registration, you do not need to carry the organ donation card (if any) with you. DH will not issue any organ donation card to you. We appeal to people who have signed an organ donation card to register at the CODR, in order to facilitate confirmation of your wish and make arrangements accordingly.

  5. If you submit your application with “iAM Smart”, you can use “e-ME” to fill in your personal information (including your Hong Kong Identity Card (HKIC) number and name in English) to complete your application with the CODR. Your identity will also be verified at the same time.

  6. Registration is voluntary and free of charge.

  7. If you need to amend your registration record after registration (for example, you want to change the organs to be donated or other personal information), you can submit a new application for registration. Your record will be updated.

Personal Information Collection Statement

Purpose of Collection

  1. The personal data provided in the application will be used by the Department of Health (DH) and authorised personnel for the following purposes:
    1. processing your application and verifying your identity information;
    2. arranging organ and / or tissue donation and transplant; and
    3. performing relevant data management.
    The provision of personal data in the process of the application is a voluntary act. If you do not provide sufficient information, we may not be able to process your application.

Classes of Transferees

  1. The personal data you provide may be disclosed to other Government bureaux / departments or relevant parties for the purposes mentioned in paragraph 1, if required. Apart from this, the data may only be disclosed to parties where you have given consent to such disclosure or where such disclosure is allowed under the Personal Data (Privacy) Ordinance (“the PDPO”).

Access to Personal Data

  1. You have a right of access and correction with respect to personal data as provided for in sections 18 and 22 and Principle 6 of Schedule 1 of the PDPO. Your right of access includes the right to obtain a copy of your personal data provided by you during the occasion as mentioned in paragraph 1 above. An administrative fee will be charged by the DH for complying with the data access request by providing a copy of your personal data.

Protection Measures and Retention of Personal Data

  1. The DH will, in accordance with the PDPO and according to the nature of the personal data collected, adopt all reasonably practicable measures to ensure that all personal data are correct, kept securely and confidentially and protected against unauthorised or accidental access, processing, erasure or other use, and will follow the relevant provisions in making corrections and giving access to data.
  2. Your personal data will be kept by DH for a reasonable period of time as may be required for the above specific purposes for which they were collected and in accordance with the relevant statutory provisions. After that time, your personal data will be erased. If the registration cannot be completed, the organ donation registration forms submitted online will be deleted by the DH.
