If you cannot provide the requested information, the DH may not be able to process your application.
If you choose to apply for withdrawal in-person or online, identity verification can be completed up front during the application process. If you apply by post or fax, upon receiving your application, staff of the DH will process your application.
In general, for applications submitted online, the withdrawal process will be completed in a shorter time. If the CODR currently holds your registration record, after your withdrawal application submitted in person, by post or fax, you will receive a notification within 14 days after submission of your application. If you do not receive the notification, please contact the CODR Office of the DH by phone on 2961 8441 / fax on 2127 4926 or by email at codr@dh.gov.hk for enquiry.
If you submit your application with “iAM Smart”, you can use “e-ME” to fill in your personal information (including your Hong Kong Identity Card (HKIC) number and name in English) to complete your application with the CODR. Your identity will also be verified at the same time.
Registration in the CODR is voluntary and a person may withdraw the registration at any time.
If you wish to sign up as an organ donor after successful withdrawal, you can submit an application for registration.
